What You'll Do
- Perform physical therapy evaluations and treatments in patients' homes across the Phoenix West Valley
- Develop and implement individualized treatment plans across adult and pediatric populations — shaped by your clinical interests
- Address mobility, balance, strength, coordination, pain management, fall prevention, and motor development across the lifespan
- Complete OASIS assessments and Medicare documentation
- Coordinate care with physicians, home health agencies, and interdisciplinary teams

What We're Looking For
- Licensed or license-eligible PT in the state of Arizona
- New graduates are welcome and encouraged to apply
- Home health experience is a plus, but not required — we'll train you
- Reliable transportation for travel between patient homes
- Strong clinical reasoning and a patient-first mindset
